Rental Form
Fill this out ONLY if you're under 18, will not be accompanied by a parent or legal guardian, and are not enrolled in multi-week programs or in a one-time group. If you're over 18, you can fill out a rental form at the resort. Please contact the Clackamas Sales Office, at 503.659.1256, if you're in a multi-week program or one-time group and have not received your rental form.
