If you're under 18 and your parent or legal guardian can't accompany you to get your picture, have them print out and sign this release.
Credit Card Authorization Form
If you're purchasing a credit card-backed product (ie. the Midweek+ Plus or the Meadows Club Card), then you'll need to sign this form.
Minor Participant Parental Consent Form
This form is intended for those under 18 who are renting equipment or taking a lesson at Mt. Hood Meadows and are not accompanied by their parent or guardian. This form must be filled out completely, signed by both the participant and their parent or guardian, and presented at the resort by the participant.
